A 55 year old male presented with:

- Giddiness since 4 months
- Dyspnoea since 1 month
- He had episodes of PND.
ON EXAMINATION:

- Pulse 80/min, regular, low volume
- BP: 90/50mm Hg, Rt arm supine.
- JVP not elevated
CARDIOVASCULAR SYSTEM:
INSPECTION:

- Apex in left 5th ICS in midclavicular line
PALPATION:
- Apex beat confirmed to be left 5th IC in midclavicular line
- Systolic thrill in right 2nd ICS.
- Carotid thrill+

AUSCULTATION:
- S1 S2 normal
- Ejection systolic murmur heard in aortic area conducted to carotids.
OTHER SYSTEMS: Normal.
