  1. A 66 years old hypertensive patient has presented in emergency room with breathlessness, oliguria, anasarca and weight loss since 1 month. He has uncontrolled hypertension since last 6 months. His renal profile is persistently abnormal since 6 months. CBC shows severe anemia, urinalysis proteinuria and ABG metabolic acidosis. USG finding is suggestive of small size kidneys.
