a) A 50 year old lady with history of pulmonary tuberculosis ten years back. She has completed full course of anti kocha medication. However it resulted in right lower lobe bronchiectasis. She had intermittent episodes of infection now & then for last ten years. At present she complains of increased frequency of infective episodes with productive cough with foul smelling sputum. It is difficult for her to even laugh or talk for long as it ends up with cough and stress incontinence. She has dyspnoea on performing accustomed activities. She works as a clerk and wishes to continue with work but is worried if she would be able to.

a) A 55 year old male, manager of a multinational bank was diagnosed to have triple vessel disease & underwent CABG three weeks back. He was discharged from the hospital after three days post operation; however he did not follow the advice of the physiotherapist. He is now referred for rehabilitation at first follow up, He is a chronic smoker not ready to quit smoking. He is trying to alter his dietary habit with some success. BMI is 30 and has low levels of HDL cholesterol. The incision is well healed but patient is unable to raise his hands above head.

b) Forty year old Sunita has undergone Right radical mastectomy three days back. Discuss the immediate and late complications that may arise and ways to prevent them. She is right handed and has heavy cooking duty as she gives tiffin boxes to ten clients in the morning and evening. Plan a rehabilitation program for her with reasoning. Why and how does she need to take care of her hand life long?
