1. A 28-year-old female presents with diffuse swel ing in the mid line of neck

2. A 64-year-old man presents to you with complaints of bilateral knee pain

for last few years. He is increasingly incapacitated now because of the
pain and difficulty walking.
a) What are your differential diagnosis?
b) What medicines would you give for pain relief?
c) What other modalities of pain relief would you suggest?
d) In osteoarthritis knee, what are the indications for surgery?
3. A 41-year-old man presents with history of trauma in the palm after a

road traffic accident. He has no other injuries.
a) What first aid would you give him?
b) What regional anaesthesia would you like to give? What nerves and
where would you block them, if he requires minor surgery?
c) What are the local anaesthetic drugs you can use for a peripheral
nerve block?
d) What precautions would you take while doing a peripheral nerve
block?
4. A 65-year-old man needs to undergo a hernia repair. What conditions

would you look for in preparation for the anaesthesia? What
investigations would you require, with reasons? He has diabetes,
hypertension & COPD. How would you prepare him for surgery?

6. A young man met with an accident. He is opening eyes to painful stimuli,

a) How would you assess this case?
b) How would you manage this case now?
7. A 62-year-old man, a known diabetic for the past 15 years, presents to

you with a non-healing ulcer over dorsum of left foot for the past 2
months.
a) What are all the various tests you wil do on this patient?
b) How wil you manage his wound?
c) As a Family Physician, how wil you prevent occurrence of another
ulcer in the future.
8. You have diagnosed a 10-year-old girl to have safe chronic suppurative

otitis media. Write the clinical features, examination findings,
investigations, treatment and complications of CSOM in such case.
9. A 65-year-old female patient presents with pain and swel ing in the left

thigh since 5 days. There is some local warmth, peripheral pulses are
normal.
a) What are your differential diagnosis in such case?
b) What investigations would you request, with reasons?
c) How would you manage deep vein thrombosis?
10. A 40 years old lady, presents with abdominal pain since 2 days which is

gradual y increasing. Her vitals- BP 100/60mmHg, pulse 110/min, RR
25/min. Abdomen has generalized tenderness and guarding.
a) Write 4 differential diagnosis you would consider with
common clinical findings in each.
