1. A 50 year old man presents with fever, fatigue, anorexia and weight loss since 2 months.

He has productive cough and hemoptysis. On examination he has hyper pigmentation of
palms and soles, a B.P. of 80/50 mm Hg and right pulmonary apical crackles. Answer the

2. A 30 year old female presents with an episode of tonic-clonic convulsions. She recently

delivered a preterm underweight baby. Patient's mother noticed a rigid posture, urinary
incontinence and tongue bite. On examination she is drowsy, arousable and complains of
head ache. Her blood pressure is 110/70 mm Hg. Answer the following.
