  1. A five year old girl was admitted with fever, chills and productive cough for five days. She had a runny nose few days prior to the onset of fever. She looked ill and her respiratory rate was 34/m n. On auscultation, there was reduced air entry and crepitations over the left lower zone of the chest. Her white cell count was 11.58 x 10ΒΊ cells/L and CRP 68.3 mg/L.

    1. Name the most probable diagnosis in this patient. (10 marks)

  2. A 58 year old man was admitted with a history of headache, fever and difficulty in sleeping of three days duration and difficulty in drinking for one day. His daughter said that the patient was bitten by a stray dog three weeks before and did not get any treatment for the dog bite. The patient looked anxious and agitated and was asking for the fan to be switched off. He choked when he tried to drink water. Two days after admission he developed difficulty in breathing. He died on the third day of admission in spite of all the care given.
